Assessment is completed through contacts with patient, family, OP providers, review with team, and review of record. Contacts all involved agencies within two business days of admission
Assessment includes evaluation of living situation, family issues and support network, significant cultural factors, current stressors, level of functioning prior to admission, psych history, and d/c planning needs.
Assessment, treatment plan and goals integrate information obtained and clinical observations. Treatment Plans are realistic for time frame of hospitalization, patient specific and goal-oriented. Obstacles to plans or outcomes are identified.
Maintains current knowledge of patient's legal status and legal issues ensures appropriate documents are available in chart and filed in a timely manner.
Screens all new assigned cases within one working days, to assess patients' living situation and identify potential disposition problems.
Coordinates discharge planning with patient, family and outside agencies as necessary through individual and family meetings, telephone contacts and providers meetings. Maintains effective and consistent communication with OP providers and agencies.
Coordinates aftercare planning with psychiatrist. Obtains assessment information from other team members and uses this information in determining an appropriate aftercare plan.
Arranges discharge plan to coincide with readiness for discharge.
Meets with patient regularly and involves patient in treatment and discharge planning.
Conducts "Transition" appointments with patients as needed and documents for record.
Attends and is active participant in weekly scheduled multidisciplinary team meetings, as observed by the Social Work Director. Provides input to team members regarding psychosocial problems, plan and goals, and discharge planning needs. Maintains effective communication with psychiatrist other team members.
Is visible on unit, available and accessible to staff/patients. Responds to pages promptly. Maintains effective communication through verbal contact with psychiatrist and unit staff.
Handles conflicts or problems in the team effectively and independently, or reports unresolved problems to the Director in a timely manner. Continues to work to provide constructive solutions to problems or conflicts even after involving the Director
Responds promptly and appropriately to complaints, concerns, requests of customers (team, patient, family, UR, outside agencies).
Makes early identification of families in need of intervention, support or education. Convenes family meetings as indicated, within time frame.
Makes effective intervention and maintains good communication with family and support network.
Rationale for and objectives of family/couples intervention is clear in treatment plan and progress notes.
Attends regular supervision meetings with supervisor, takes initiative to reschedule meetings if necessary. Takes initiative in individual supervision to identify and study problem cases, review the delivery of casework services, and discuss inter-disciplinary team work.
Attends and participates actively in in-service training opportunities.
Attends continuing education programs in the community to enhance clinical skills
Progress notes are documented every other day.
Progress notes clearly and concisely document nature of social worker's contacts, discharge planning interventions, outcomes, and continued plans.
Progress notes are problem oriented and related to treatment plan.
Conveys a positive attitude toward job duties and new ideas as monitored by the Coordinator. Assumes additional responsibilities as requested or assigned, and completes assigned tasks accurately and on time. Maintains a professional demeanor by adhering to published attendance standards; contacts supervisor relative to reason for absence and expected date of return in accord with department policy. Arrives at work punctually.
Obtains and maintains license.
